Chandler Hall, a Quaker based long-term care residential and community service organization, funded by Friends Foundation for Aging and the Scattergood Foundation, developed a model to assist providers of long-term care services with the integration of behavioral health approaches and services into their long-term care operations. Behavioral health and long-term care have often operated in separate geographic, regulatory, and financial "silos," with integrated care ("geropsych") often considered the step-child of both worlds.
The model, which may be replicated with or without the workforce element includes:
1) Workforce and Staff Development (to increase staff's skills and knowledge re: behavioral health)
2) Integration of Health and Behavioral Health Professionals (to improve communication and focus on behavioral health) and
3) A Positive Pursuits Program for skilled nursing and personal care Residents (to integrate the preventive and mood-enhancing benefits of pleasurable activities with a "person-centered" care environment).
